First step is to create a Create a Client Card at portal.gmgsavings.com using the normal methods. (see also: How Do I Add a Client Card? )

Then, submit the Prospect Form by logging into your Hub at my.strydeadvisors.com and clicking on Financial Services under Service Channel dropdown. 

Then scroll down to the Health & Prescription section to click on the Prospect Form.
